A five-year multicenter prospective cohort study has been conducted using this cancer registry system since 2010 (J-WEB/EGC) and the result was published in DEN. The first one is a CONNECT-G trial to investigate the usefulness of endo-clip connecting dental floss during gastric ESD that has a potential efficacy making a better view by traction with DFC. The second one is an RCT to compare the second-generation NBI with white-light imaging for detection of early gastric cancer .
We have completed a prospective cohort study for the possibility of chemo-radiotherapy for high-risk rectal submucosal cancer after endoscopic resections. We are playing a leading part to conduct a multicenter single-arm study for the possibility of CRT for highrisk rectal submucosal cancer after endoscopic resections. When this study result shows a noninferiority of CRT compared to surgery, we can propose a new non-invasive treatment strategy for rectal T1 cancer patients. Early Detection and Intervention Center for Schizophrenia will be established during 2013. Our aim is to promote early detection of patients with schizophrenia, to offer appropriate clinical care for those in the early phase of illness, which is called the "critical period," and to assist clinical and basic research to improve our understanding of diagnosis and intervention. We provide appropriate care based on detailed examinations, implementing "shared decision making" and multidisciplinary approaches at our specialty outpatient clinic.
Another closely related field of study I am engaged in is problems specific to English usage in the presentation, both written and oral, of medical research. This work is being pursued in collaboration with other members of the Japan Society for Medical English Education. Institutes and Hospitals work together closely to establish a system for developing advanced medical treatments, including clinical studies on muscular diseases. With an eye to the future, we offer rehabilitation appropriate for the stage of disease. Patients with insufficient expectoration of sputum and other respiratory disorder symptoms may receive pulmonary rehabilitation and home mechanical ventilation. We conduct a consultation aimed at devising a support system for achieving better home care.
In order to put an end to such turmoil, the Ministry of Health, Labour and Welfare in 2007 issued the end-of-life care guidelines mentioned above, which was followed by a number of medical societies’ end-of-life care guidelines. How should we help patients to achieve their own way of living during the final stage of life narr atives? That is, it is vital to acknowledge that different people have different approaches. Years ago, the mainstream way of thinking was that if you have any means to prolong the biological life of a patient, you should do so; if you do not do so, then it is disregard for life.
In this lecture, she will share her broad knowledge and precious experience about her long challenges for radiological protection of children who had/will have radiosurgery including radiation diagnosis. The guidelines formulated by the Ministry of Health, Labour and Welfare are compatible with Advance Care Planning , which is attracting global attention. ACP recommends that the patient, family and medical service providers should undertake appropriate communications when making 花小金井 歯医者 medical decisions. ACP includes Advance Directives , such as the living will and POLST (physician orders for life-sustaining treatment), but the documents need to be revised accordingly through the process of discussions and dialogues. This is based on the idea that both patients and medical service providers should share medical information and information about patients’ life narratives in order to deepen mutual understanding and proceed with treatment plans together.
Instead, I suggest that you should use the term end-of-life care and shift your mindset to thinking together with patients about care at the final stage of their life narratives. We are collaborating with NCCH research institute and Osaka University and performed fecal metagenomic and metabolomic studies on samples from a large cohort of 616 participants who underwent colonoscopy to assess taxonomic and functional characteristics of gut microbiota and metabolites. Our large-cohort multi-omics data indicate that shifts in the microbiome and metabolome occur from the very early stages of the development of colorectal cancer, which is of possible etiological and diagnostic importance. A nationwide cancer registry system has been developed for early gastric cancer treated with EMR/ESD.
In this case, the doctor bears no legal responsibility for the termination of life-prolonging treatments. This is a familiar scene for family attending the deathbeds of elderly people in Japan, but now is the time to reconsider whether or not it is beneficial for the patients themselves. The needle of an intravenous drip needs to be stuck into the arm of a patient many times. We often see elderly patients whose arms have turned brownish as a result of multiple injections of intravenous drips.
In the Gastrointestinal Endoscopy Division, more advanced and technically difficult endoscopic treatments such as endoscopic submucosal dissection are being used in place of conventional endoscopic mucosal resection not only for early gastric cancer, but also for superficial esophageal and colorectal neoplasms. In addition, educational activities are an important part of our division's activities with many Japanese medical students, residents and staff physicians as well as approximately 130 overseas post-graduate physicians attending our training courses annually. The materials I use for this research range from students’ writing to school and university textbooks and other writings in or on the English language published in Japan.
This study is collecting many T1 CRC cases from over 70 institutions all over Japan and will bring important information on the management of T1 cancer. We are currently enrolling our patients in several multicenter randomized controlled trials . Introducing schools, kindergarten schools, and pre-schools with curriculum and education of country of origin in mainly an English speaking environment for foreigners. Applied to personal dose monitoring, this introduces additional value to the use of passive dosemeters. Under optimized conditions, CO2 and UV treatments combined in an original two-phase etching technique increase the total number of tracks by more than 230 % in nuclear track detectors, and the maximum diameter of the tracks by more than 60 %, compared to the standard routine processing. A series of tests have been performed on active personal dosemeters , with the emphasis on their use in interventional radiology/cardiology radiation fields. The results lead to some recommendations and attention points, mainly focused on the influence of pulsed fields and the lead apron backscatter. The current state-of-the-art in the solid state dosimetry are briefly reviewed and some vision on future trends of advancements in solid state dosimetry technology and their various applications are presented.
The dosimetry of megavoltage small fields can be challenging due to lateral charged particle disequilibrium, detector volume averaging effect, and high dose gradients. 3-D radiochromic plastic dosimeter, PRESAGE, can be used to acquire small-field beam data as well as patient-specific IMRT/VMAT QA precisely and accurately. The international intercomparisons for calibration methods of passive dosemeters IC2017calm and IC2018calm were organized by EURADOS to study the precision of calibrations of participating dosimetry services and institutes. Against the background of this kind of legalization efforts are precedents under which doctors were held to have legal responsibility, such as the Tokai University Hospital case. In such cases, doctors were convicted of murder for injecting patients lethal drug.
We have started a collaborative study with the Departments of Endoscopy, Colorectal Surgery, Gastric Surgery, and Pathology and Clinical Laboratories, the NCC, Research Institute, the University of Tokyo, and the Jikei University School of Medicine. A nationwide cancer registry system has also been developed for early CRC treated with ESD. A five-year multicenter prospective cohort study has been ongoing using this cancer registry system since 2013. A total of 2,066 patients were enrolled in this multicenter cohort study and this should be the largest cohort study in colorectal ESD in the world. We have organized several multicenter study groups to evaluate the efficacy and clinical impact of newly developed endoscopies and medical devices prospectively. Our Endoscopy Center was authorized as one of the WEO Centers of Excellence by the World Endoscopy Organization and got an award during Gastro 2017 in Hyderabad.
We have conducted a multicenter prospective cohort study on colorectal neuroendocrine tumors since January 2017. This study is expected to provide new important findings on colorectal NETs. For more accurate endoscopic diagnosis of gastrointestinal disease, we are utilizing the NBI and BLI systems that enable us to narrow the spectral transmittance bandwidth of the optical filters used in the light source of electronic endoscope systems.
